Community Based Program Manager

The Program Manager will be responsible for developing and supporting programming for 3-5 individuals to help them achieve goals that will contribute to creating the life they want with support through adult programming.

Responsibilities

  • Provide case management and skills training support to clients in areas such as career exploration, hard and soft employment skills, college campus inclusion, communication, decision-making, social skills, transportation goals, community safety and activities of daily living.
  • Develop programming and curriculum on a case by case program that allows opportunities for clients to develop skills to achieve their goals in a safe and controlled environment.
  • Review case records and programming and curriculum on a regular basis to ensure compliance with You're With Us! standards.
  • Participate in implementing and monitoring curriculum and treatment plans, completing all necessary programmatic documentation and reporting significant concerns in a timely manner.
  • Develop and run group activities in the community.
  • Act as an advocate for You're With Us! clients and their families.
  • Develop and maintain positive relationships with guardians, family members and other service providers.
  • Perform other related work duties as assigned by senior You're With Us! staff.

Required Qualifications

  • Associates Degree in Human Services or related field, or 4-5 years of supervisor experience within a day or residential program for individuals with disabilities.
  • Direct Care experience working with individuals with Intellectual/Developmental Disabilities and at least one year of CBDS experience.
  • Excellent organizational, communication, and writing skills.
  • Demonstrated ability to work both independently and collaboratively with a culturally diverse population.
  • Ability to write clear, concise and accurate documentation.
  • Ability to lift 30 pounds as needed.
  • Valid driver's license and acceptable driving record is required.
  • Current First Aid, CPR and Crisis Intervention certificates or be able to obtain these within thirty (30) days of hire.
  • Pass criminal background check (CORI) and National Fingerprint Background Check.

Preferred Qualifications

  • Bachelor's Degree in Human Services or related field preferred.
Part-Time Position

Campus Case Coordinator

The campus case coordinator will be responsible for program development and implementation of You're With Us! on our college campuses.

Responsibilities

  • Maintaining schedule of inclusion opportunities of participant in group activities.
  • Helping with onboarding of group and participant.
  • Scheduling of support services needed for each participant.
  • Marketing of You're With Us! on campus.
  • Recruiting and creating new opportunities within groups on campus.
  • Compiling data on each visit by participant.
  • Help facilitate Positive Experience Design.
Part-Time Position

Peer Mentor

The peer mentor will report to the campus case coordinator and play a vital role in supporting participant engagement and inclusion activities.

Responsibilities

  • Supervise participant engagement and activities within college group.
  • Compile data around inclusion for each activity.
  • Help brainstorm on problem solving around barriers that pop up within Positive Experience Design.
  • Assess how support services are helping with inclusion.
  • Supply content for social media around opportunities.
